Notice is hereby given that Somnatek Sleep Health Center, operating at 465 Upper Riverdale Road, Suite 2, Harrow County, Georgia, has voluntarily ceased clinical operations as of September 18, 2014.

Facility license HRWCO-FAC-0031 is hereby considered expired as of the date of this notice. The license will not be transferred or reassigned without a new application to the Harrow County Health Department.
